Our Health Clerk cannot dispense any over-the-counter or prescription medication(s) without a physician's authorization form on file. Authorization forms are available by clicking on the link at the right or in the school's Health Office. Students must submit their prescriptions to the Health Office where the Health Clerk will supervise the administration of all medication(s). Students may not carry any type of medication(s), including aspirin and ibuprofen.
P.E. Notes
A parent/guardian may write a note excusing a student from P.E. class for a maximum of two consecutive class sessions. If a student cannot participate in P.E. more than two sessions a medical doctor's note will be required. The doctor's note must state the duration and specific restrictions/limitations and/or modifications. Students will not be released to go back to P.E. prior to the date on the doctor's note unless a new doctor's note is brought to the office.
