Medications at School
Our Health Clerk cannot dispense any over-the-counter or prescription medication(s) without a physician's authorization form on file. Authorization forms are available by clicking on the link at the right or in the school's Health Office. Students must submit their prescriptions in the original packaging to the Health Office where the Health Clerk will supervise the administration of all medication(s). Students may not carry any type of medication(s), including aspirin and ibuprofen.
P.E. Notes
A parent/guardian may write a note excusing a student from P.E. class for a maximum of two consecutive class sessions. If a student cannot participate in P.E. more than two sessions a medical doctor's note will be required. The doctor's note must state the duration and specific restrictions/limitations and/or modifications. Students will not be released to go back to P.E. prior to the date on the doctor's note unless a new doctor's note is brought to the office.
Medical Devices
Any student who is required to have a hard/soft cast, splint, sling, or brace or an orthopedic device such as a walking boot or knee brace, or who needs the use of crutches, walkers, or wheelchairs must provide the school nurse with a note from a medical provider stating the diagnosis and restrictions/precautions that must be taken in school. All physical activities such as P.E., lunch time activities, or extra curricular activities will be restricted until a note is received from the medical provider clearing the student to resume to normal activities. A copy of the medical provider's note will be provided to P.E. teachers and yard staff.
