All incoming 7th grade students must show proof of having received the following immunizations otherwise they will not be allowed to complete the check-in process, nor attend the first day of school, until proof is obtained by the school.

If your child has medication they need to take at school please click on the "Request to take medication" form on the right. Fill it out, have your doctor sign it, and submit the form and your child's medication to our front office.
